Pricing: Labs, tests and medication prescriptions can be submitted to your insurance if you have one. However, please note consultation time is not covered by insurance. You can review pricing using the link below.
Consultations: Standard consult time is 1 hour.
Consents: Prior to your first appointment, please review and submit the signed consents using the link in your appointment confirmation text/email or via Patient Portal
Health History Intake Form: Complete before your appointment within the patient portal.
Recent Labs: If you have recent lab results (<1 year), you are welcome to bring them with you for the first appointment or upload them to the patient portal

We have two options for booking your initial consultation.
Option 1: Follow the link below: BOOK APPOINTMENT and schedule an 'Inital new patient consult.' The link will promt you to create your patient profile and complete required consent and intake forms.
Option 2: Call our office at 321-895-3017 and we can assist you in creating your patient profile. You will receive an email/text with links to the required consents and intake forms.
Please note we must receive complete consent and intake forms prior to your appointment or it may be canceled.
Your first appointment with Kelsey is approximately one hour to allow for a thorough history intake and review of current concerns and symptoms. To assist with diagnosis and treatment plan, please submit or bring copies of the following:
Recent lab results (within the past year) to help establish a baseline, if available.
Recent Mammogram, DXA scans, or other relevant testing/imaging results (within the past 3-5 years).
You will receive several appointment reminders.
If you need to cancel and/or reschedule, please provide 3 full business days advance notice per policy to avoid incurring an automatic $100 cancellation fee.
