HOW TO BECOME A NEW PATIENT

We value transparency and want our patients to know what to expect when they join our practice.

We have two options for booking your initial consultation.

Option 1: Follow the link below: BOOK APPOINTMENT and schedule an 'Inital new patient consult.' The link will promt you to create your patient profile and complete required consent and intake forms.

Option 2: Call our office at 321-895-3017 and we can assist you in creating your patient profile. You will receive an email/text with links to the required consents and intake forms.

Please note we must receive complete consent and intake forms prior to your appointment or it may be canceled.

Create your patient portal access to complete the required forms and health history PRIOR to the appointment. If you cannot complete it at home, please arrive 10 minutes prior to your appointment to complete in office.

• Bring or submit to Patient Portal and recent lab results (within the past year) to help establish a baseline if available.

• Bring or submit to Patient Portal recent Mammogram, DXA scans, or other relevant testing/imaging results (within the past 3 years).

You will receive several appointment reminders.

• If you need to cancel and/or reschedule, please provide 3 full business days advance notice per policy to avoid incurring an automatic $100 cancellation fee.
