Needing a Refill?
To streamline your care and ensure medication safety, all prescription refill requests must be submitted directly by your pharmacy. Please contact your pharmacy when you need a refill, and they will send us the request electronically. We are unable to process refill requests made directly by patients.
Cancellation and No-Show Policy
Please be aware that missed appointments or cancellations with less than 3 days notice will incur a $100 late fee. Repeated no-shows or late cancellations may result in dismissal from the practice. We appreciate your understanding and cooperation in helping us provide timely care to all patients.
